Your NDIS plan is a set of possibilities, and it's your life that decides what they become. Here's how we can support you, at home, in your community, and everywhere in between.

01 - Support at home

Your home, your routine, your way

Home is where support should fit around you, never the other way round. We help with the everyday things that keep life running, exactly how you like them done.

This includes complex and high-intensity personal care, delivered by workers trained and supervised under the clinical oversight of our registered nurse Clinical Lead. If your support needs are significant, we're equipped and registered to meet them well.

This can include

  • Personal care and daily personal activities

  • Complex and high-intensity personal supports

  • Household tasks like cleaning, laundry and meal preparation

  • Building everyday living skills at your pace

  • Support with routines, planning and organising your day

02 - Getting out and about

Your community, on your terms

Life happens out in the world: with friends, at activities, in the places you love. We support you to get there, take part, and belong.

Whether it's a regular group, a one-off event, or somewhere you've always wanted to go, our support is built around what you want to do, with transport to get you there and back.

This can include

  • Support to access your community, events and activities

  • Transport to appointments, work, study and social life

  • Group and centre-based activities

  • New and creative ways to participate that are designed with you

03 - Community nursing

Clinical care, at home, from people who know you

Nursing care works best when it comes to you, delivered by someone who takes the time to understand how you like things done.

Our community nursing is always led by a registered nurse Clinical Lead, so the care you receive at home is planned, delivered and reviewed to a professional standard, with you involved at every step.

This can include

  • Nursing care delivered in your home by a registered nurse

  • Clinical assessment and care planning, done with you

  • Training and supervision of your support workers for clinical tasks

  • Regular reviews so your care keeps up with your needs

04 - Supported living

A home of your own, with the right support in it

Everyone deserves a place that feels like theirs. We support you to find it, set it up, and live well in it, including through supported independent living (SIL).

From help with tenancy and accommodation through to shared living arrangements and around-the-home support, we work with you to build a living situation that fits your life and your goals.

This can include

  • Supported independent living (SIL)

  • Help to find and keep suitable housing

  • Support with tenancy obligations and living arrangements

  • Shared living support and daily tasks in your home
